Change instructions on task completion evidences #144

Reworded instructions for

  1. Responsive Web Design
  2. Intro to JS
  3. DOM

Added code-review.md stub file.

Also other minor changes are made.

NB! Please, check instructions in js-dom.md as this task contains practical subtask that requires code review. All details on code review per se are to be added to code-review.md. The task description only mentions that code review is required and mentor's approval should be added to student's course repo README.md.

Partially resolves #142

Next step is to complete other tasks evidence instructions based on this PR review.
