Add schema qualification to the piggy index.xml report web page #21

The procedures that show up on the report web page are not schema qualified.

I'm okay with this. What do you think of omitting the schema if all procedures belong to the same schema, and displaying the schema when they don't? I'm trying to keep the names short to prevent the name from line wrapping.
