get rid of old node and io versions. add v4 and v6

Looks like some failing tests on versions 4 and 6. I'll give a test locally.

@shaunwarman I think this test failures are because of the unfinished tests (missing t.end statements) I fixed recently along with other documentation changes. Perhaps just updating this branch will make this work :)
