mfa_serial is not allowed anymore in cross-account profiles.
A default profile should be recommended.

mfa_serial should work in roles and long-term credentials (2.1.1). We actually should (on top of that) support passing in an MFA over commandline and environment on top of that (since using an MFA with long-term is not really standard in shared configs).
