Adds Issue templates #214

This pr adds template for bugs and feature requests. Adding this so that we have structured and enough details when other contributors raise some issue.

I think you can open this PR against the main branch. It won't affect any source files. thoughts???

We usually raise prs against mvp_demo and then on releases merge them to main branch. @dinogun can we raise this one directly on main?
