Scan Barcode(PDF417) for autofill via phone/computer camera #51

Feature is to allow folks to autofill the COUNTY, Date of Birth (only applicable for miami-dade currently), and driver license number. Florida Driver licenses use PDF417/ISO 15438, zbar seems to be a popular solution.
