There were a couple of problems here that were both silently lost before I exposed the exceptions:

Deleting a key from the HashMap while iterating over it caused an exception about concurrent modification.
It was possible for the HashMap to intentionally be emptied while iterating over it, which could cause an iteration to point to null.
