FIA data ingest (minor)

Minor fixes for the FIA data ingest

[x] remove control vocabulary flag if not defined in annotations. For example: https://github.com/ktoddbrown/SoilDRaH/blob/8b7408f886e4f29f27dbc0b0d513dc96cc245a9f/data/FIA_Annotations.csv#L19
[x] add study_id to the annotations table of FIA and study information including package url, citation, and when the annotations were last updated.
