Refactor /pkg/provisioner into a coherent sub-dir hierarchy #109

The /pkg/provisioner directory was hastily written and could use a cleaning up. Helper function files should be moved to a subdirectory, (./internal/ would be preferable) to separate responsibilities of the packages.
