Add new charts for 1.25 release team shadow applicant survey

The release team's shadow applicant survey is being changed for 1.25 and this may open up the possibilities for new charts to be published with the report.

• [ ] Discussed which charts should be added to the report for 1.25
• [ ] Completed Implementation to generate the report

The shadow application form has been updated and send out to the shadows

new fields for all:

• How many times have you applied to join the Release Team? *

new fields for newcomers:

• Will you be able to attend Release Team meetings? * (now a selection)
• Will you be able to attend Burndown meetings? * (now a selection)

new fields for returners:

• Which Release Team roles have you served in? *

I think we can create new charts reflecting exactly the new fields listed above.
