add one more file or folder "Request Failed" #501

Hi,

i tried to solve it by myself, but no chance.....

here is my issue:
i have a foldertree with some pdfs, docs, jpgs and subfolders.
everything works fine.
but when i'm uploading (with mollify and filesystem)one new file or i'm 
creating another folder, i get a "request failed" message.
the file appears in the filesystem as new file but in mollify i get the 
errormessage.

attached i send you the "php_error.log" and the "mollify_debug.log"

Sorry, there were no errors in the logs. Please clear the logs, reproduce the 
error and then get the logs (you should find line with ERROR).

Did I understand correctly, only upload causes the error? And is the file is 
actually uploaded ok? Does this happen on uploading any file?

at first, thanks for your reply.
i have token a look at the log-files for any lines with ERROR. but no luck.

i have a empty folder on my filesystem. this folder i assigned in mollify.
everything fine. 

i create a subfolder with the windows explorer or mollify(whatever)and refresh 
the view in my browser, the folder appears in the browser. everything fine.

i can create a second and a third subfolder or i can copy a second or third 
file to that folder but at once, when i refresh the view in the browser it 
appears the error message "request failed" (The logfiles i attached when the 
error occurs).

maybe the screenshots helps you to understand my issue.

I deleted the log attachments, because they can contain sensitive data.

Could I get access to your site? I suppose there can be some error happening 
before it gets to mollify at all, but these should end up in the logs as well 
if PHP logging is properly set.
