Hint on New Family Profile page is confusing #116

What steps will reproduce the problem?
1.Log in as Mary789
2.Choose Family Profile
3.Choose Create New Family Profile

What is the expected output? What do you see instead?

The resulting form has hints in the fields. The one for the date field just 
says Month/Day/Year. It doesn't tell the user WHICH date to enter. 
Please use labels and text to provide additional information. It should say 
"Patient Date of Birth (Month/Day?Year)"
