Room requests are inserting into roomreservationactivity table with a FamilyProfileID of 0 #124

What steps will reproduce the problem?
1. Login with mary789 and password1.
2. Go to request room and type last name: miller
3. Choose Scott Miller.
4. Fill out form with dates December 15, 2012 and December 20, 2012 and 
diagnosis of cancer, submit.

What is the expected output? What do you see instead?
When you look at the roomreservationactivitytable you should see a new record 
with a FamilyProfileID of 2. However, you'll just see a 0.
