Form fields are not saved after unsuccessful New Family Profile submission

What steps will reproduce the problem?
1. Login as a SW with username "Mary789" and password: "password1"
2. Select "New Family Profile"
3. Leave one or more fields empty.
4. Hit "Create Family Profile" or press enter.

What is the expected output? What do you see instead?

The form is returned, with all values set to their defaults (empty, with 
labels).
Ideally, the system would return the form pre-populated with the values the SW 
attempted to enter, and any missing fields highlighted with red or some other 
highly visible indicator.

Please use labels and text to provide additional information.

This problem makes testing issue 77 and
