Form fields are not saved after unsuccessful room request submit #99

What steps will reproduce the problem?
1. Log in with mary789 and password1.
2. Go to request room and in the family look up last name: miller.
3. Choose Scott Miller and create room reservation. 
4. The field information should be prefilled. Enter in a begin date: january 23 
2013. Do not enter an end date. Press submit.

What is the expected output? What do you see instead?
You should see the page refresh with the prefilled information that was there 
before along with an error message at the top of the page stating why the 
submit was unsuccessful. However, the page refreshes with the error message and 
all the fields except PatientDiagnosis are filled with some html erroring with 
an accompanying error message saying the field variables are undefined.

Please use labels and text to provide additional information.
