Site doesn't work after uploading 'default.php' in templates->mod_login #12

What steps will reproduce the problem?
1. Uploading of 'default.php' causes total crash of the site
2.
3.

What is the expected output? What do you see instead?
Working site with captcha.
Nothing - blank screen

What version of the product are you using? On what operating system?
Joomla 1.5.7, Joomla Captcha 300

Please provide any additional information below.

Look system requirements of title page. If you use low versions PHP than 5.0 
and 
Joomla Captcha less than 4.0 Stable. Then it is necessary for you to bring 
adjustments in a code independently.
