A full and update document required #170

What steps will reproduce the problem?

1.All document like README* or CHANGELOG were very old.
2.We require a correct and full document to let more people use grub4dos.
3.Maybe one READEM.txt and one Reference.txt in utf-8 format will be helpful.
4. maybe http://manpages.ubuntu.com/manpages/trusty/man1/txt2man.1.html will be 
helpful.

What is the expected output? What do you see instead?

What version of the product are you using? On what operating system?
