implement extended ordered lists features for textile #311

http://txstyle.org/doc/9/numbered-lists includes two features that aren't 
implemented in pandoc.

The first one is to start the first numbered list with a different number than 
1, just as in this sample:

#5 fifth item
# sixth item

The second feature is to be able to resume numbering from last numbered list. 
Here is a sample:

# first item
# second item

#_ third item

Could you implement both features for textile in pandoc?
