Improve location structure #29

Make a few improvements around how locations are described:

Place "geo" inside a "location" attribute for confinement information.
use "toLocation" for confinement breaking credentials

Also, describe confinement breaking credential a bit more formally:

Use a specific credential type of "ConfinementInterruptionCredential"
Place "confinement" inside the confinementInterruption object, since an interruption is about a specific confinement. This is different from the semantics of health-related credentials.
