handler is an instance of a handler interface and not callable itself, so handle()
method should be called on it

Closing; this looks like it was supposed to be a patch, but it's been reported as an issue. (discrepancy likely due to transition from old organizations).
Fixed a typo in $handler invocation in the documentation for creating-middleware.md
