Arducopter ignoring modes #504

Arducoper NG or ACM? ACM
What steps will reproduce the problem?
1.Unable to reproduce consistantly
2.Usually happens on the second battery of the day.
3.after 8 to 10 minutes Copter ignores inputs from mode selector switch 

What is the expected output? What do you see instead?
Expect change of modes.  Instead it has stuck in "Auto" "Alt Hold" and "Pos 
Hold" on separate occasions.

What version of the product are you using? On what operating system?
APM2 Arducopter 2.8 FW, Mission Planner 1.2.14 mav 1.0, Win7 x64, Turnigy 9x 
flashed with er9x, HK TR6A V2 receiver, uBlox LEA-6 GPS,3DR Telemetry 900mhz, 
mb1200 sonar.

Please provide any additional information below.
This has happened in three separate locations with different hardware 
configurations - different gps, no sonar.  Copter seems to ignore channel 5 
inputs.  Latest incident was in stabilize, landed, took off, switch to auto 
mode, flew a lap around the waypoints then i noticed that i couldn't change 
modes.  It just kept flying around.  I had to hand catch it and pull the 
battery to avoid anyone getting hurt.  Log file shows near the end that channel 
5 was providing input and the board was reading it but it just stayed in auto.  
Attempts to unarm were also un successful.  I didnt have the idea to try to 
change modes in the mission planner, telemetry is new to me so it hadn't even 
crossed my mind.
 I will attach the logfile.

It would seem that it was ignoring modes because the cli says radio not 
calibrated.  The same forgotten radio calibration issue persisted after a 
factory reset of the apm2 and fresh install of arducopter Quad 2.8.

Resolved.  Re-calibrated radio, and set mode 6 to stabilize to avoid fly-offs 
in the event that it happens again.

Can you enter the CLI and place the output of setup - show in this report?
I'm looking for rc5min value.
