Services Inquiry Form

As a potential customer, I want to know how to get in contact with them, so I can request one of the services provided.

Acceptance Criteria:

• [ ] Form link is accessible on Service page
• [ ] Form contains all necessary fields: name, email, phone number, type of service, message and preferred method of contact.
• [ ] All fields on the form must be completed before the user can submit the form.
• [ ] User is clearly inform of success submission of form upon completion
• [ ] After completion, message is displayed so user knows what to expect (contact via e-mail, phone, etc)

Tasks:

• [ ] Implement a booking inquiry form on the Service page.
• [ ] Implement HTML validation on the form to fit the requirements above.
• [ ] Create a success page or success notification after submitting the form

considering merging Service Inquiry form with Events Sign-up form to minimize cognitive load and complexity for user, but that would require some Javascript for conditional rendering form fields.
