[WIP] Use stable anchors in table of contents. #37

Github-flavoured markdown renderers automatically add anchors to headings, this removes the org-generated ones (which change every time you export) in favour of the standard.

FSF copyright because org-gfm-headline is derived from org-md-headline.

issue: Doesn't handle headlines with sequence tokens in them, because GFM renderers (obviously) include those in the anchor.
