Refactor Components and Commands #85

Refactor LaunchDarklyCompletionItemProvider to its own separate class
Add CMState object for cleaner handling of configuration changes
Move commands that were in providers.ts to their own files
create generalCommands.ts for easier reloading of commands on config changes
