Something about PFAA being loaded at the same time as Railcraft, adds a 2nd version of the creosote bottle from Railcraft. I think I saw it do the same thing for the RC Steam Bottle too, but I didn't specifically test for that. This also occurs without NEI, when using the search tab in creative.
