update existing issue

• fix https://github.com/homoluctus/gitrivy/issues/59
• update existing issue if there is one, otherwise create a new one
• a duplicate issue is a issue with the same label and the same image
• in case there is already an existing image, update the body
• add option to fail in case vulnerabilities were found
