Add more variables related to heart health #71

New harmonised ELSA G.2 release contains many more chronic health variables related to heart health. Some of these would be very nice either as predictors for mortality, disability etc. but also some would just be good outcomes on their own. Lets include:

[x] Angina: RwANGINE
[x] Heart Attack: RwHRTATTE
[x] Congestive Heart Failure: RwCONHRTFE
[x] Heart Murmur: RwHRTMRE
[x] Abnormal Heart Rhythm: RwHRTRHME
