ARM Custom Form: Notes and Limitation Statements

I think this is done for now. One can select a Limitation as a Note type and supply the value. Will want to remove the ability to Select an existing note.

Limitation notes can be added, but there are several issues to address with notes:

• The note value cannot be added when creating the note. You have to go to the note page and add the value there.
• The note value appears under property group "Instance Parts and Accompaniments" on the note page.
• Remove ability to add an existing note - must create new - as per Steven's comment. (Oddly, when you click to add a second note, there is a message saying that there are no existing notes in the system from which to choose. It's a bug, but one we need not worry about since we don't want to do this anyway. Maybe a search index problem?)
• Needs a list view to display note value rather than note entity on the resource page.
