Update testnet exit market maker and Bridge UI to support CAS #150

Bounty

Once testnet is upgraded to CAS release, it is time to move all the infrastructure to use CAS bitmaps for exit proofs.

Scope

update exit market maker deployment to the latest version with CAS support. It is already implemented, just needs to be deployed and tested.
update Bridge UI to support CAS proofs for exits. Implemented here, needs to be deployed and tested

bridge UI fast and slow exits working with CAS-activated testnet
