Uploaded documents in customforms have wrong URLs #2983

It looks like the links are the full server directories for the files, not the URLs to the files on the specific site: image

Furthermore, we don't encode the URLs properly (in the user-facing and admin facing interfaces, I think), so URLs with unsafe characters (e.g. / and ") don't work.
