Apply for URN status #54

We need to create an application - based on appendix A of this document: https://www.ietf.org/rfc/rfc3406.txt.

We should have Stuart and Jeanne review the application when we are finished.

The URN we are proposing is: "urn:ctid:[uuid]"

We need to collect a list of participating organizations to include in the app, as well as a technical detailing of how our namespace is managed.

Registrants should send a copy of the registration template (see Appendix A), duly completed, to: urn-nid@apps.ietf.org

I pasted some info by analogy with the example they have in the document, and marked in orange the places which are left to fix - this is just the example information that they give in the URN Namespace Definition Mechanisms RFC.
