[Patch] Put songinfo in the window title (HIG) and don't change it 2 times during song change.

Fixes:

1) Put only "~title~version - ~people" in the title instead of "appname -
title" so you can actually read the title if QL is in the windowlist.

2) Depend on the song-started signal instead of library-changed. Stops
annoying title switch to appname and back during song change.

In my opinion QL is a document-based application..
I don't see the point of showing "Quod Libet - This is a..." in the window list.
Rhythmbox, Banshee, Exaile, VLC behave this way.

I looked at this for a while and frankly didn't care for losing "Quod Libet" in 
the
title, but... *shrugs*
