pending CONDs not removed #16

When adding a few loops, pending CONDs show up that do not get committed, nor removed. Looks like maybe these should have been rejected, but they're not removed from ledger._pending.

Maybe I'll work on #15 first, since the code is not structured enough at the moment to convincingly avoid bugs like this.
