This is a great project. I suggest adding some framing about for whom and in what contexts this checklist is relevant. Specifically, I'm worried about organizing contexts in which designers/participants may be fully aware of the sensitivity of their research.
Basically, I'd suggest emphasizing that this tool should be applied very broadly and possibly adding some common sense factors that might designers should make use of it. For example: "Does your project touch on issues that some users may feel private about?" and "Is it possible that your participants might share information that would embarrass them or cause them danger if it was shared publicly?"
