Separate CHANGELOG.md to integrate better #35

By moving the existing 'Change Log' section out of README.md into a new CHANGELOG.md file it will be given its own tab on the Extension page in VS Code and and entry in the Resources sidebar section on its Marketplace page.

I will submit a PR.
