Add support for metadata in the import script #24

Because we infer data type from the dataset and we don't support parsing a data dictionary, extracting metadata like ICD10 codes and variable descriptions is impossible.

I think that adding option to parse a data dictionary is not the way to go and that it is best to let users do this manually in the Excel file for now.
