provide installation docs

The installation process has grown into quite a hurdle for non-developers. Several steps should be documented:

CouchDB installation (with links for Windows users)
CouchDB database creation
initial setup (copy example.yaml to config.yaml; provide a quickstart script?)
how do modules work?
