Switch bug-report issue template to YAML #542

Switches to GitHub form-schema YAML syntax for bug-report issue template, to make reporting bugs easier and more efficient.

NB:.github/ISSUE_TEMPLATE/bug_report.md would need to be deleted for this to take effect. If desired, I would be happy to change the other two issue templates from Markdown to YAML.
