Supplements in web.xml

What steps will reproduce the problem?
1.i have copy the web.xml contant as you mention here as it is.
2. and i typed in the url the first one that is 
"http://localhost:8080/jay/servlet1/monitoring" . 

What is the expected output? What do you see instead?
 the expectation is the same output as that in the "http://localhost:8080/jay/monitoring"
because in the second one it gives proper output.
What version of the product are you using? On what application server, JDK,
JavaMelody 1.43.0
jdk1.6.0_05
apache-tomcat-6.0.36
operating system?
windows 7 enterprise 32bit operating system

Please provide any additional information below.

I understand from this issue that "http://localhost:8080/jay/monitoring" works 
and gives report,
and that "http://localhost:8080/jay/servlet1/monitoring" does not work and does 
not give report,
where "servlet1" is a servlet in your webapp.

This seems expected to me. As said in the user's guide, the report is expected 
at "http://localhost:8080/jay/monitoring", where "jay" is your webapp's context.
So what is the problem?
