Add options for "no insurance" and "other" to insurance list #188

How we do this is still TBD, but adding the issue so we address it when everyone is back from holiday.

We need to sort these options out before we can start on pricing. Adding Ben to get his thoughts.

other would be used to capture plans that we do not support yet and would prevent the user from finishing onboarding.

no insurance which I see as an equivalent to cashcould be a seeded as a practice-specific plan. Every interaction triggers a look up against a fee schedule (price list of clinical services).

@bsiscovick: Has this been addressed in the cohort selection, or does this need to be addressed prior to launch? Cross referencing with https://github.com/leo-health/leo/issues/373, since they look to be related.

i don't think this will be an issue for the early cohorts - they are already members of the practice so presumably their insurance will be listed.
