With default orthography profile #2

The default profile was created by

adding a iter_raw_lexemes method to Dataset
running lexibank orthography mcelhanonhuon
copying (and fixing) the profile to etc/

Then lexibank makecldf mcelhanonhuon was run again.

The last commit

reduces the amount of code in the dataset by a couple of lines
makes the CLDF more future proof (I think), because it decouples local IDs from Concepticon development.

Since now local concepts are used as parameter IDs, changes in Concepticon mappings for the concept list will only result in changes in cldf/parameters.csv, not in cldf/forms.csv (due to changed foreign keys).
