Add support for relations #6

Add relation support for the following formats:

BioC (JSON and XML)
PubAnnotation JSON
Brat and BioNLP

Relations in PubTator, which are not connected to entity mentions in text, cannot easily be converted to the other formats and are therefore currently not supported.
