I have only found one workaround, which is to quit the game (hope it autosaved recently!) and restart.
My guess is that some sort of background throttling is put in place when you move the game to the background, and that throttling is not properly removed if you are away "too long". (Perhaps if a certain event happens in the meantime, like an autosave.)
It doesn't seem to matter:
If you "pause" the game in a UI screen (using e.g. Esc) or stay AFK "in-world".
Whether you're using a built-in MacBook Pro screen or an external monitor.
Observed: the game runs at less than 1fps.
Expected: the game resumes playing.
