Firebase integration #5

This sets up the app for a multi-user SaaS, using Firebase as the backing database.

Users can sign up via invite codes, configured in Firebase backend.
Nightscout MongoDB URL's are fetched from the Firebase datastore.
Annotations are stored in a Firebase datastore, no longer in the browser IndexedDB.
