e.g. The INT16 mixer doesn't seem to work at all. I just get noise when I attach an INT16 mixer to an INT16 voice.

We should have an example that exhaustively tests sample/mixer/voice combinations, like we have for pixel formats. We will probably want a program to generate the mixers too.
