Sync to latest upstream and general core updates #83

Update core files to latest version 1.23.0. cdrom stuff, state handlers and vdc+vde optimizations not included, the rest is update and file layout restored for easier upstream sync if needed in the future.

A few general updates as well to make core updated with latest changes like input bitmasks.
