Health care access sources? #2

Consider any ways that access to health care can be built in to a metric. What data sources may be available? Initial thoughts are we can look at percent of population insured in a zip, percent of population who have a primary care physician, etc...? Other considerations for how to proxy access to healthcare and hence access to tests?

Would medicare spending be a good proxy for this? I would think this would be made public by zip code.
