Ready for 0.9.0 #119

This pull request includes:

all documentation updates,
support for altering settings pre-rewiring (needed as now commands run after extraction/rewiring),
a fix for command defaults expansion,
new code to support "extraction-version" with the onLoad-driven extraction,
a few other minor changes.

The code has been tested with the extended community build (including plugins), and all is ok. We are ready for the dbuild 0.9.0 release.
