Document how to install a plugin and configure lightblue to load it at runtime. #34

Associated with https://github.com/lightblue-platform/lightblue-rest/issues/162.

No longer are "plugins" going to be included in lightblue-rest and will need to be installed and configured individually. This should be documented.
