INSTALL.md: clarify nginx instructions + add ToC #17

Closes #16 and #18. I added a note about the readonly.macaroon in the .env file if that works.

Nginx, Option 1, and Option 2 are un clickable in the ToC. May need to massage the output of teh script a bit (if that's what you used) so it all renders properly.

Fixed! just had to remove special characters from the markdown links..
